DMA served as the evaluation advisors for North Suffolk Community Services' CCBHC grant funded by SAMHSA. DMA also provided project management services to assist with the implementation of the CCBHC model. DMA regularly reviewed performance data, follow-up interview rates, and progress in achieving grant objectives. DMA also prepared progress reports detailing achievements, barriers, efforts to overcome barriers, implementation challenges, and Quality Improvement efforts.
